"Tsuki to Hanataba" is the fifth single released by Sayuri. It was released in three versions: a limited CD+DVD edition, a regular CD edition, and a limited CD+DVD anime edition. Each edition comes with one random character card. The title track was used as the ending theme for the anime Fate/EXTRA Last Encore. The single peaked at #10 on the Oricon charts and charted for nine weeks, selling 20,466 copies.

- This is a shortened version of the song which was used as the end credits for the anime Fate/EXTRA Last Encore. It was included on the Anime Edition of the single as track #3.

- This is an alternate arrangement of the song where Sayuri plays guitar and sings to her own accompaniment. It appeared as track #2 on the Anime Edition of the Sayuri x MY FIRST STORY single, "Reimei".
