"Koi no Super Ball / Form" is the 28th single released by aiko. "Koi no Super Ball" was used as the "Calpis Water" commercial song and "Form" is the theme song for the film Hankyuu Densha Katamichi 15fun no Kiseki. Since the movie is set in the Kansai region, there is a special Kansai version released at the same time. For this version, "Form" is the first track, and it features a different cover jacket, booklet and a special colored CD case.
